This is a dementia awareness information session for those living with Dementia, their loved ones and carers.

The Primary Care Assessment Team is a five&nbsp;day&nbsp;a week community-based service operating Monday to Friday from 8.30am to 5pm.

Dementia Friendly Swansea

We, Dementia Friendly Swansea, are dedicated to improving the quality of life for individuals living with dementia in and around the Swansea, South Wales area. Through our Dementia Hwb project, we provide a range of support services and activities aimed at assisting both individuals with dementia and their caregivers. Our initiatives include pop-up information services and forums. We also offer signposting to practical assistance, respite and breaks, social services, and access to NHS services. Our work is made possible through collaboration with partners such as Swansea and NPT Carers Centres, Alzheimer’s Society, Age Cymru West Glamorgan, Swansea Bay Health Board and Swansea and NPT Councils, and we are proud to have become a registered charity in 2023 to further our impact.

Our efforts have resulted in a growing network of over 70 members, including healthcare professionals, small businesses, and volunteers, who come together bimonthly to discuss and implement dementia-friendly projects and initiatives. We aim to support not only those who have been diagnosed with dementia, but also individuals who are concerned about their memory, as well as those who care for loved ones with dementia. Through our work, we strive to create a supportive and understanding community, providing essential information, advice, and practical assistance to improve the lives of those affected by dementia.
