Youth Horizons is a short break provision for young people aged 11+ in Hammersmith &amp; Fulham with SEND. The Saturday term time club runs from 12:00pm–3:00pm, offering a safe, inclusive space supported by specialist staff.Activities include:<ol><li>SEND multi-sports (inclusive and adapted sessions)</li><li>Virtual reality experiences</li><li>Community trips</li><li>Outdoor activities</li><li>Creative arts and crafts</li><li>Music sessions</li><li>Peer support and social time (group activities with specialist staff)</li></ol>Youth Horizons supports wellbeing, independence, and confidence while providing families with a valuable short break.

The Access Hub multi sport sessions are an inclusive program designed to reduce isolation, improve well-being, and promote physical activity for young people with Special Educational Needs and Disabilities (SEND). Through adapted multi sport activities, creative arts and virtual reality opportunities young people will gain confidence, build friendships, and enhance their physical and mental health in a supportive and accessible environment. The sessions are for young people aged 6 - 18 years from Hammersmith and Fulham. We provide breakfast snacks, lunch and refreshments throughout the day. Please note sessions are group sessions and 1:1 support is not available.

Lets unite for autism is a charity driven by the lived experiences ,offering support, advocacy and empowerment for lifelong success.

We provide
1.The provision of advice, guidance, support and information to those affected by autism.
2.Provision of recreational and leisure time activities designed to improve life skills, capacities and capabilities.
3.Advancing the education of the public in autistic spectrum conditions, in particular among those who work with people who have autistic spectrum conditions.

Plinth provides detailed descriptions of each provider and event, including age suitability, accessibility arrangements and availability, helping you make informed choices.&nbsp; CancellationsYou will receive an email reminding you of the activities you have booked for your child a week before delivery. If you need to cancel, please do so via Plinth and contact the provider. We ask that you give at least 24 hours notice so the place can be given to another young person. SEND and AccessibilityOur Holiday, Activity and Food Programme providers strive to accommodate all children and young people. We encourage parents and guardians to contact individual providers to discuss any specific requirements your child may have. This communication is crucial to help us understand how we can best support your child and ensure their safety and enjoyment. Specialist SEND ProvisionWe collaborate with specialist providers who are experienced in working with children with complex and profound needs. These providers offer specialist programmes and activities designed to cater to the specific requirements of children with disabilities or other additional needs.
