Best Start Family Hub Registration Form (TEST)

Address

Split from the combined 'Postcode and address' field on the document. Matches the existing 'Address' field.

Email

Semantic and name match to the existing 'Email' field. The label clarifies it should be a parent/carer email for child members.

Form Completed By

This is a separate field for the person filling in the form on behalf of the member (e.g. a staff member or carer). No existing field matches this metadata field.

Full Name

Exact match to the existing 'Full Name' field. This is the member being registered and must be first.

Under 18

No existing field matches this. While there is a Date of Birth field, this is a direct yes/no question used on the form rather than a derived value.

Member Type

No existing field matches this. Captures whether the member is a parent, carer, child, or other.

Expecting a Baby

No existing field matches this. Specific to this Family Hub registration form.

Gender Same as Sex

No existing field matches this gender identity question. The existing Gender field is used for sex, so this follow-up question requires a new field.

Phone

Semantic and name match to the existing 'Phone' field. The label clarifies it should be a parent/carer number for child members.

Postcode

The document has a combined postcode and address field. Split into separate Postcode and Address fields per UK conventions. Matches existing 'Postcode' field.

Gender

The document has a 'sex' field and a separate 'Is their gender the same as their sex?' field. The existing schema has a single 'Gender' field. Mapping 'sex' to the Gender field is the closest match. The follow-up gender identity question is captured separately as a new field.

How Did You Hear About Us

No existing field matches this referral source question. Created as a new field.

Date of Birth

Ethnicity

Exact match to the existing 'Ethnicity' field under DEMOGRAPHICS. The document marks this as optional, matching the equalities information section.
