Description

Join us for this quarters health and wellbeing forum, we will be looking at the Integrated Neighbourhood Teams, the neighbourhood forums and an update on the VCS strategy and state of the sector report for the North East London VCSFE collaborative.

We will have a presentation from Tim Hughes who is leading on developing the integrated neighbourhood teams in Tower Hamlets.

The Integrated Neighbourhood Teams is a new initiative in the NHS designed to bring together a range of professionals, including social prescribers, GPs, practice nurses, mental health professionals and the voluntary sector. In order to deliver joined up preventative care at a neighbourhood level to address the specific needs of local populations and tackling inequalities.

Here is the NHS confederation document on the case for change.

Sam Crosby will be giving us an update on the work of the neighbourhood forums in PCN 1 and 2.The programme is looking at health literacy in young people and access to interpreting and advocacy services to improve communications with communities.

Vicky Scott the borough representative will be giving an update on the state of the sector survey and the VCS strategy. And on the themed representatives and how the sector can support resident voice in the integrated care meeting structures.
