EPIC Health Programme: Long Term Health and Care Needs Strategic Group Meeting aims to provide a space for anyone who wants to play a role in the preventing and improving the Long-term health and care needs of residents in City and Hackney.

Topic: Exploring collaboration opportunities and solutions to challenges in Long Term Health and Care.

Meeting Objectives:

• Explore collaborative opportunities between Voluntary and Community Sector VCS, health professionals, and local authorities to improve long-term health and care outcomes.

• Agree on key actions for the VCS, health professionals, and local authorities to strategically align to improve support and appointment access for individuals with long-term health conditions.

• Identify 2025 strategic priorities for the Long-Term Health and Care Strategic Group.

• Identify how can we better support families in managing the challenges associated with long-term and care needs?
